I have a super simple project in php form to capture 5 text fields in browser, store to mysql table, place fields inside a premade text document, add a header image and footer image, maybe adjust font to be more formal looking as an A4 or legal size document, and save it as a pdf and email it to me... (Budget: $10 - $30 USD, Jobs: HTML, MySQL, PHP)
